gost server and ghost vm's

I have a server that had a major hardware failure we had to rebuild the server. when we rebuilt the server it was issued a different hostname and ip address. We added the server to the pool and was able to deploy a vm on the new server.
the issue is the old server is listed in the pool, we can not remove the server from the pool, on top of that in XOA it is showing 2 vm's running on the ghost server.
- how can one clean up the ghost vm's?
- remove the reference to the old server form the pool?
